In general, you can expect to receive most of your 2025 tax year forms, starting in January 2026. We will update the tax center and this schedule with specific details as they become available. Please check back here for the full list of forms and important dates.
You can access your tax forms by logging in to your online account.
For annuity and life insurance customers:
- Log in
- You'll be taken to the new Tax Center (you can also access it from the left menu by clicking "Tax Center").
- Select the year and view and print your documents using the table below.
For retirement customers (If you are an Empower customer, visit empowermyretirement.com Opens in new window):
- Log in
- Click “View Details” for the Prudential plan for your Group Annuity benefit (there may be more than one account if you have multiple Prudential products).
- From the menu on the left-hand side, choose “eDocuments.”
- “Tax Forms” are the first section: Click “View/Print Prudential Tax Forms” to access your forms.
- Log in
- You'll be taken to the new Tax Center (you can also access it from the left menu by clicking "Tax Center").
- Select the year and view and print your documents using the table below.
You may need tax forms for each if you have multiple Prudential products, such as life insurance, annuities, or retirement accounts. To ensure you receive all your documents:
- Verify that your contact information and mailing address are up-to-date for each account. You can update these online or by contacting customer service
- Tax forms are sent on a set schedule, which can vary by product. You can request a copy if you have not received it by the expected date. Log in to your account and go to the tax documents section to view and print your forms.
- If you prefer paper copies, contact customer service and specify which products you need forms for. We'll send them as soon as possible.
Your form may have been sent electronically, as we are moving to digital communication to reduce paper waste. Check your email (including the spam folder) for a digital copy.
If you still need a paper copy, contact our customer service team. We'll verify your mailing address, check the status of your form, and send a paper copy if needed. For annuity tax forms, you can also use our chatbot to request a 1099-R by mail. You’ll need to provide your Contract Number, Date of Birth, and the last four digits of your Social Security number.
